Assumption of Risk *

I do hereby assume full responsibility for any and all damages, injuries, or losses that I/we may sustain or incur, if any, while attending or participating in The Nutcracker and/or during any visit to Myers Ballet studios or Northeast Ballet Company. I/we hereby waive all claims against the Myers Ballet Studio and Northeast Ballet Company, its instructors, agents, staff or partners of said studio, individually, or otherwise, for any and all claims for injuries or damages that I/we might sustain. I understand that there is a risk of injury associated with participation in The Nutcracker. I/we certify that I/we are in good physical condition and have no disabilities or other ailments that may hamper my/our participation. I certify that all of the information provided on this document is correct and true.

Media Release *

I hereby grant to Northeast Ballet Company, and their respective licensees, successors and assigns, the right and permission, with respect to photographs and videos taken of me or the minor named below on whose behalf I am signing, and with respect to any printed matter in connection therewith, to do the following: 1. To include such photographs and videos in all media, and in advertising, publicity and promotion thereof. 2. To use my name or the name of the minor on whose behalf I am signing, in connection with the foregoing. I hereby release, discharge and agree to indemnify and hold harmless Northeast Ballet Company and their respective heirs, legal representatives, licensees, successor and assigns, from all claims and demands whatsoever arising out of or in connection with the foregoing, and waive any right to inspect or approve the same.

Statement of Commitment *

I understand that all participants in Northeast Ballet Company's Nutcracker production are considered to be serious and dedicated to the production. I understand that lack of appropriate behavior, attitude, and attendance is grounds for immediate dismissal from the production. Technical week and all studio dress rehearsals are mandatory (December 1, December 11-15). All absences must be approved by Miss Myers.

Casting Contract *

If a specific role is not listed it is because a final casting decision will not be made on certain roles until additional rehearsals have been held. All cast members and parents must realize that although every effort is made to keep the roles that have been cast the same; there may be times when it becomes necessary to make changes. Reasons may be artistic, risk of potential injury, or the result of illness to name a few. In any event, changes are made at the discretion of the artistic director and are final. If you or your child(ren) are not going to participate in the production, please call/email ASAP to let us know.

Casting Contract *

Being a part of these Nutcracker performances at such an exciting time and is a wonderful opportunity for your dancer! It is something they will remember for a lifetime! Additional Nutcracker cast performance opportunities include:, a Proctors’ Education Department (daytime) School Show & two Proctors full performances. Casting for each show will be determined by the artistic director. Northeast Ballet Company is a non-profit organization that relies upon parent volunteers to help in all aspects of its productions. There are no costume fees required from cast members. However, as with any organization, a commitment is required . We need and expect your contribution of time and skills to keep this production organized, the backstage area safe and orderly and to make this a positive experience for everyone.

Ticketing Information *

Northeast Ballet does not sell any tickets to any of these productions. We suggest you purchase tickets as soon as possible since they sell out quickly. We cannot change casting to accommodate purchased tickets. If your dancer has a part that is double cast (one cast for Sat. show and another cast for the Sun. show) it will take several weeks of rehearsal before we know which cast is doing which performance. As soon as a decision is made, we will let you know. Please wait to purchase your tickets until the decision has been made, or perhaps organize a ticket purchase and exchange with others. There will be professional photographers and videographers documenting these rehearsals and performance, which will be made available to cast members. Therefore, no photos or videos are allowed in rehearsals or backstage , especially in costume.

Absence Statement *

1. Attend all rehearsals. Remember: An Absent Dancer Impacts Every Dancer! Please, always provide a written note about any absence as soon as you are aware of it, and know that any absences may result in casting replacements. While we have scheduled designated days and times for the various roles, there may be times when we need to change them. We will make every effort to keep the schedule as it stands from the beginning. We will always inform you of any change that may be required, in addition to having all schedule update postings at www.northeastballet.org

Attire *

Appropriate attire is expected at all times. Female dancers should wear leotards, pink tights, ballet slippers and/or pointe shoes and hair up in a bun. Male dancers should wear black ballet slippers, black tights and tee shirt. For all performances you must have new tights, appropriate make-up & accessories. Costumes are provided .
